To provide a thorough inpatient client assessment to evaluate all medical and psychological needs, and to provide quality services to the terminally ill patient and their families to focus on support and care in a time of need.

To provide support for the patient's emotional, social and spiritual needs, as well as medical symptoms as part of treating the whole person.
OUR PROFESSIONAL STAFF
Attending Physician
Attending Physician remains the patient's primary care physician and works as a member of the hospice and palliative team.
Medical Director
Medical Director certifies the attending physician's prognosis, leads the hospice and palliative team in developing a plan of care, and provides consultation to the attending physician.
Registered Nurses
Registered Nurses evaluate the patient's condition during regular scheduled visits, coordinate the plan of care, and teach the family proper patient care techniques and use of medications.
Social Workers
Social Workers provide emotional support for the patient and family, meanwhile counseling in dealing with end-of-life issues, helping with advanced directives.
Certified Home Health Aides
Certified Home Health Aides provide personal care services designed to increase the level of comfort.
Volunteers
Volunteers provide non-medical friendly visit or respite to patient and family in the areas of emotional support and encouragement.
Chaplains
Chaplains initiate contact with patient and family to determine needs and provides spiritual support. Works as liaison with clergy of individual's choice.
